How do you choose a primary care physician (PCP)?
Frequently you choose a primary care physician (PCP), commonly known as a family practioner, internist, or pediatrician, during open enrollment with your insurance provider. If you did not, you can simply call UPN at 847-763-7266 and our member services department will help you select your UPN PCP. Remember, health maintenance organizations (HMO’s) require you to choose a PCP for managing and coordinating your health care.
How do you select a different PCP?
If you have BlueAdvantageSM HMO and/or HMO IllinoisSMplan, you can simply call UPN and we can make the change for you. Should you wish to change to a new PCP, please contact our member services department at 847-763-7266, on or before the 25th of the month, to be effective with the new PCP the first of the next month. If you have a Humana or Harmony HMO plan, you call them to make the change using the 800-number on your membership card. Regardless of what HMO plan you have, you can always call UPN for guidance at 847-763-1700!

How do you select a different Obstetrician/Gynecologist (OB/GYN)?
If you have selected an OB/GYN affiliated with Unified Physicians Network and wish to change your Obstetrician/Gynecologist (OB/GYN) woman’s primary health care provider (WPHCP) to another UPN OB/GYN WPHCP, simply call member services at 847-763-7266.
How do you see a specialist?
Access to a specialist requires a written authorization obtained from your PCP. The specialists address and telephone will appear on the authorization so that you can schedule the appropriate appointment. You are responsible for being aware of the expiration date of the referral, the type of service and the number of visits the referral authorizes. If you decide to see a specialist without getting a referral from your PCP, you will be responsible for the charges.
What should you do if you receive a claim?
You are financially responsible for “copayments”. If you should receive a bill for any cost greater than a copayment for authorized services, please call UPN at 847-763-1700.
How do you get an insurance card?
UPN does not issue insurance cards; you must call your HMO insurance provider for a new or replacement card. The card should name the medical group along with the applicable “copayments”.
Who will need your medical records?
Please be sure to obtain your medical records from your previous physician as soon as possible. If you need a copy of your medical records, contact your current primary care physician – you may need to complete a request for release of medical records form. Your new PCP can assist you with the transfer of medical records to their office.
